Показатели жесткости сосудов в зависимости от полиморфизма генов АСЕ и AT2R1 у больных артериальной гипертонией Республики Мордовия

Abstract

Цель. Изучить показатели жесткости сосудистой стенки в зависимости от полиморфизма генов ренин-ангиотензиновой системы (РААС) у больных артериальной гипертонией (АГ) Республики Мордовия (РМ). Материалы и методы. Обследованы 162 больных АГ русской и мордовской (эрзя, мокша) национальностей без клинических проявлений атеросклероза и ишемической болезни сердца, а также 92 нормотоника, не имеющих родственников первой линии с повышенным уровнем артериального давления (АД). Проведено физикальное обследование, лабораторные (включая молекулярно-генетические) и инструментальные методы исследования с регистрацией скорости пульсовой волны в аорте (СПВА или PWao) и индекса аугментации (ИА или Aix) с использованием артериографа TensioClinic (Tensiomed, Венгрия). Результаты. При изучении показателей сосудистой жесткости в зависимости от полиморфизма генов РААС (АСЕ и АТ2R1) у больных АГ Республики Мордовия были выявлены ассоциации неблагоприятного генотипа DD гена АСЕ со СПВА и ИА у мужчин с АГ национальности мордва-эрзя и русской национальности, а у женщин национальности мордва мокша и русские ассоциация прослеживается только с патологическими значениями ИА. При анализе показателей ригидности сосудов с полиморфизмом гена AT2R1 получены ассоциации генотипа СС данного гена с патологическими значениями ИА у больных АГ женского пола русской и мордва-эрзя национальностей.

The purpose is to explore the stiffness of the vascular walls in relation to genetic polymorphism of the renin-angiotensin system (RAAS) in patients with arterial hypertension (AH) with the residence in the Republic of Mordovia (RM). As for materials and methods the study included 162 Russian and Mordvin (Erzya, Moksha) patients without clinical manifestations of atherosclerosis and coronary heart disease, and 92 normotensive patients with no first line relatives with elevated blood pressure (BP). We conducted a physical examination, laboratory (including molecular genetics) and instrumental methods of examination with the registration of aortic pulse wave velocity (PWV) and augmentation index (AI or AIX) using arteriography (TensioClinic, Tensiomed, Hungary). The results of the impact of RAAS genetic polymorphism (ACE and AT2R1) on vessel stiffness indicators for AH patients in Mordovia revealed associations of negative DD genotype of ACE gene with SPVA and IA agency for AH Mordovian-Erzya and Russian male patients. For Mordovian-Moksha and Russian female patients the association can be traced only with pathological values of IA. When analyzing the performance rigidity of vessels with AT2R1 genetic polymorphism we obtained the associations of CC genotype of this gene with IA pathological values for AH Mordovian-Erzya and Russian female patients.
