K-06 Caractéristiques des patients de la cohorte NADIS sous antirétroviraux depuis plus de 10 ans

Abstract

Objectif Decrire via la cohorte NADIS les caracteristiques des patients VIH1+ ayant ≥ 10 ans d’antiretroviraux (Pat10) pris en charge en France en 2007. Methode Etude transversale de la cohorte constituee des donnees, anonymisees et soumises a un controle de qualite standardise, des services hospitalo-universitaires specialises dans l’infection a VIH utilisant NADIS, dossier medical informatise de gestion en temps reel de la consultation medicale. Resultats Au 30 septembre 2007 parmi les 8 598 patients en cours de suivi et traites par antiretroviraux, il y avait 45,4 % ( n = 3 901) Pat10, de mediane de seropositivite a 12,1 ans. Les Pat10 avaient une mediane de 8.2 lignes antiretrovirales et de 5 recours dans les 12 derniers mois. Leurs dernieres medianes de CD4/mm 3 etait a 543 [248-838] et de charge virale a 1,7 [0,8-2,5] log. En regression logistique, par rapport aux patients avec vs. 68,4 %), plus jeune au diagnostic VIH1 (mediane = 30 ans vs. 34), en stade sida (30,1 % vs. 27,1 %), avec un nadir de CD4 bas (CD4 vs. 87,1 %), une co-infection hepatite B/C (32,2 % vs. 22,3 %), en echec viro-immunologique (19,3 % vs. 8,8 %), avec des effets secondaires (28,9 % vs. 22,9 %) a l’instauration du dernier traitement. Ils avaient moins souvent un traitement actuel comprenant 2 INTI + 1 INNTI (19,6 % vs. 28,3 %) et une charge virale indetectable (83,8 % vs. 87,2 %). Conclusion La cohorte NADIS issue d’un systeme d’information reactif permet d’obtenir l’historique des lignes antiretrovirales et montre qu’apres 10 ans d’exposition aux antiretroviraux et avec un nombre de lignes antiretrovirales eleves, les patients VIH1+ conservent un etat immunologique et virologique proche de celui des autres.
