By 2070, 30% of people in Europe are estimated to be aged >65. While the Social and Health Care (SHC) sector makes up 10% of total employment in the EU, estimations forecast an increase by more than 830,000 new jobs in the next 10 years. In this context, multi-professional teams, assessing users’ needs and shaping user-centred health and social community-based SHC plans, have been identified at EU level as top priorities.TEAMCARE project aims to enhance the responsiveness of SHC systems to current societal challenges targeting a specific existing mismatch between the skills currently characterizing SHC Professionals working in multi-professional teams and those skills actually demanded by the public sector to shift to resilient and sustainable systems centred on community-based services, such as transversal, soft and digital skills.The main expected output of the project will be an EU Curriculum for “Specialist in Community Based Interprofessional Teams (CBIT) for person-centered care”, based on an Integrated Framework of Competences and supplied with Guidelines for the instantiation in different countries. The Curriculum will be multidisciplinary, inter-professional, based on the main EU VET standards and supporting micro-credentials. These results will be tested through 4 HE pilot courses which will be implemented in 4 countries (IT, GR, PL, IR), thus testing Curriculum flexibility. An Integrated Platform supporting SHCPs’ formal and informal learning will be also released as well as Open Contents. Recommendations and a Roadmap targeting Decision Makers will be delivered and a scaling-up strategy will be implemented.The Alliance involves 12 full partners from 6 EU countries (Ireland, Italy, Greece, Poland, Belgium, Austria). Different stakeholders across EU will benefit from the project: SCHPs, HE and VET providers, private and public social and healthcare Institutions, decision makers, and final service users.

Compared to the general population, CCS represent a vulnerable population as they are at an increased risk of developing health problems, known as late effects, resulting in excess morbidity and mortality. Many survivors are unaware of their personal risk for specific late effects, which reduces their ability to manage their own follow-up care. Similarly, their treating healthcare professionals (HCPs) lack information about care required for CCS and access to treatment data from their childhood cancer. The Survivorship Passport (SurPass) is an innovative, digital tool, developed in previous EU-funded projects, that can be used to overcome these knowledge gaps to improve people-centred long-term survivorship care. Importantly, end users (CCS, HCPs) are integral to the research, represented by three key stakeholder networks (PanCare, SIOP Europe, CCI Europe). PanCareSurPass will conduct a robust assessment of the implementation of the SurPass by first conducting a pre-implementation study in six countries (Austria, Belgium, Germany, Italy, Lithuania, Spain) representing three infrastructural scenarios in Europe. Ethical, structural, organisational, economical, national, local, privacy issues, health systems, and particular national circumstances will be taken into account throughout. An Implementation Strategy will be developed and the SurPass will be updated and validated before use in an implementation study in the six countries. The study will look at a range of outcomes including CCS activation and empowerment, CCS/HCP satisfaction with the tool, feasibility and health economics. Based on the results of the study, a Prediction Model will be developed to promote and support future implementation of the SurPass across Europe.

Despite this, no standardized Professional Profile nor training path are defined at EU level. ENhANCE targeted an existing mismatch between the skills offered by nurses working in PHC and those actually demanded by the health care sector. To be noted that the recent pandemic has made even more evident the (already existing) need of FCNs, to decentralize our PHC on the territory.<< Objectives >>Main objective was to increase the specialization level of nurses working in PHC regarding Family and Community Nursing skills. Moreover, the project aimed to foster the development of FCN curricula based on a formalized EU profile drawing on WHO and EU recommendations. Ultimately, the goal was to enhance the shift from old PHC models to new ones.<< Implementation >>Implementation encompassed the development of:- A EU Professional Profile for FCN - A EU Curriculum for FCN - Guidelines supporting VET designers and teachers to design effective training for FCNs- An Open Online Tool supporting practice sharing for FCNs- A European e-learning for VET teachers in the field of nursing- Free Open Contents targeting VET teachers - Design documents of 3 localized curricula in Italy, Finland and Greece. - 3 pilot courses in Italy, Finland and Greece.<< Results >>- 150 graduate nurses were trained according to the EU Curriculum and became FCNs; more than 30 already got employed. Others will be employed soon- Nurse professional associations and Regulatory bodies across Europe can benefit from the EU Professional Profile for recognition and mobility of the profession - VET providers and teachers acrosso Europe can offer innovative and standardized training for FCNs
