    Main goals: We propose the first long-term cohort study in Asia in assessing the effects of long-term and short-term air pollution exposures under different environmental conditions and lifestyle sub-groups in Hong Kong on cardio-respiratory mortality and hospitalisation. Methods: We will analyze data from a cohort of 66,820 older persons enrolled in the Department of Health Elderly Health Service between 1998-2001. Their socio-demographic characteristics, health status and lifestyles were r ecorded at baseline. Follow-up of mortality and hospitalization outcomes will be extended to 2012 to give a mean follow-up of 12 years (currently 9 years) (Appendix 2). Concentrations of PM10, NO2, SO2 and O3 will be obtained from 14 monitoring stations, and PM2.5 from 4 stations located throughout Hong Kong. Monthly PM10 concentrations across geographical locations will be derived from remote satellite sensing data from NASA, and converted to a 1x1 km2 resolution (Appendix 3). Daily concentrati ons at geographic locations will be determined by a combination of local monitoring and satellite data. After geo-coding residential addresses, the estimated ambient concentrations at the addresses will be used as proxy measures for exposures. (Appendix 4). Cox regression modelling with time-dependent covariates for short-term effect and time-independent covariates for long-term effect assessment will be used.
