It affects memory, thinking, orientation, comprehension, calculation, learning capacity, language, judgement and significantly interferes with a person’s ability to perform daily activities leading them to long-term care needs. There are many different types of dementia being Alzheimer the most common type contributing to ≈70% of the cases.World Health Organisation estimates 48 million people affected worldwide, number that can reach 76 million in 2030 and almost triple in 2050 to 135.5 million. Only in Europe, according to EU-28 reports, it is estimated that by 2030 nearly 14 million will have some form of dementia, rising to over 18 million by 2050. The fact that dementia is strongly associated with age makes population ageing the main factor associated with the increasing prevalence of dementia, affecting in large scale the southern Europe.Informal carers are the most important source of care for people with dementia. In Spain (such in most partner countries) about 80% of the care is provided by family, friends and neighbors, with over 70% of informal carers being women. Intensive caregiving is associated with a reduction in labour force attachment for caregivers of working age, higher poverty rates, and a 20% higher prevalence of mental health problems. Many EU countries have implemented policies to support family carers with a view to mitigating these negative impacts. The ""Spanish National Plan on Alzheimer's and other dementias 2019-2023"" recognizes as fundamental the support to informal caregivers of dependent people in their homes and the reinforcement of the mental health component in the networks of Primary and Continued Health Care. A similar approach is included in the national dementia strategies of all the countries involved in the project.There is often a lack of awareness and understanding of dementia, resulting in stigmatization and barriers to diagnosis and care. Left unaddressed, dementia could represent a significant barrier to social and economic development.<< Objectives >>Having in mind the problems & gaps identified, the project main OBJECTIVE is:> Improve the well-being of people living with dementia and of the people that cares for them, helping to create an environment where all (people with dementia, caregiver's and communities in general) can be better prepared to address the overall care required to enjoy a better overall quality of life. This general objective is achieved by creating and developing a pilot and replicable innovative community-based care ecosystem in order to stimulate public awareness and information, to increase formal and informal caregivers’ capacity, and to support and encourage carers and persons with dementia type diseases in their daily life and action.In total accordance to the recognition of dementia as a public health priority (2017-2025, Global Action Plan response to dementia adopted by EU and World Health Assembly), “reMIND” project aims to improve the lives of people with dementia, their families and caregivers, while decreasing the impact of dementia on communities with a focus on multisectoral collaboration, equity, human rights, empowerment, prevention, and service integration across the care continuum.To this extent, a set of SPECIFIC OBJECTIVES are defined to address the NEEDS of the Target groups (public in general, communities, caregivers, caretakers, entities of the social care and well-fare):> Increasing dementia awareness, inclusion & friendliness.Reducing the risk of dementia> Improving diagnosis and care> Supporting dementia carers> Enhancing health information and care systems for dementia > Fostering research and innovationTRANSNATIONALITYTo achieve the objectives, which are an important common cause, transnationality is set for assuring a wider EU perspective on the reflection & discussion on project topics, on sharing & exchange of knowledge & best practices, and where entities with homologous & complementary background can allocate relevant learnings, expertise & experience in project’s development and thus on the envisaged innovation. Further, it will allow building a pilot network for supporting community-based care ecosystem & the best dissemination of results at larger level.<< Implementation >>Along 24 months, a partnership composed by public and private institutions from Portugal, Spain, Italy, Greece and Poland, will be responsible to address project objectives through the implementation of a sequent and interconnected plan of activities, involving and benefiting directly more than 1000 people (> 500 with less opportunities) and more than 200 caregivers and staff. Indirect beneficiaries, namely through dissemination, communication measures, web-based tools, and mobile APP is expected to reach a much larger number, not less than 5000.> reMind Caregivers: A Capacity Building Programme (training for trainers, training for caregivers, and technical seminars) oriented to informal, formal and professional caregivers from the community and relevant organisations. It aims to purposefully increase and adjust knowledge, skills and capacity for consistent, systematic and reliable care supporting towards caretakers well-being, but as well towards caregivers own personal and psychological well-being.> reMind Information & Awareness: a complete communication campaign (online, face-to-face, events, and reMIND mobile App) through which the community, its institutions, families and people in general are informed and sensitized about facts and notions about the disease, its impacts in the affected persons, families and society in general, factors and behaviours of risk (prevention); tips and practices for understanding the disease and for caring about people with dementia; and about services available and the entities that can give support.> reMind Dissemination: Having in mind the pilot nature of the project and being its rationale strongly representative of the needs of many other communities all over Europe, a solid dissemination strategy is then fully justified, useful and pertinent. The partnership is highly committed in the development of this strategy, seeking to support the transfer and the appropriation of the knowledge, tools and practices by third parties that can benefit from project’s experience. As a whole, it implies the creation of a framework for lobbying, with focus on influencing integrated caregiving and well-fare community-based systems and policies at local, national and EU level.“reMind” partners believe enthusiastically that project proposal has the power to change positively the communities social care and well-fare systems and the issues addressed. This motivating idea is the compelling force behind all of “reMind” products and results. With this in mind, and with the intention of supporting communities, its organisations, caregivers and caretakers, the project should result on a free global community-based caring model, accessible for all.<< Results >>1) reMind INFORMATION & AWARENESS: A set of massive public communication campaigns (online, face-to-face, events, publications, press, media, and reMIND mobile App). The campaigns will be divided:> 1ST EDITION: directed to the community, its institutions, families and people in general, through which they are informed and sensitized about facts and notions about the disease, its impacts in the affected persons, families and society in general, and having a strong focus on prevention by addressing in a comprehensive way the factors and behaviours of risk; > 2ND EDITION: directed to the community, its institutions, families and people in general, but mainly to caregivers (formal, informal and professional) and caretakers. This 2nd edition will focus as well the community based social-care and well-fare system being implemented (Dementia Inclusive & Solidary Community) in the frame of the project (under reMind Community result);2) reMind CAREGIVERS: A complete Capacity Building Programme oriented to informal, formal and professional caregivers from the community and relevant organizations; increase and adjust knowledge, skills and capacity for consistent, systematic and reliable care supporting towards caretaker’s well-being, but as well towards caregivers own personal and psychological well-being. The “reMind Caregivers” Capacity Building Programme will be implemented in 3 different but interconnected lines:> reMind Trainers: a cycle of 2 training actions will be held at transnational level. These activities are specifically designed for trainers (Training of Trainers) and the respective training contents will be developed under the respective Intellectual Output (IO2) and implemented through the learning, training, teaching activities C1 & C2. The participants (trainers from each of the partner regions and communities) will be later on responsible for training the caregivers;> reMind Formal & Informal Caregivers: These activities, to implement locally at each partner region, will be led by the trained trainers above, and will be specifically designed for formal & informal caregivers. Again, built under IO2 it will be implemented through the LTT C3, C4, C5, C6, C7.> reMind Professional Caregivers: These activities, implemented in the frame of several events to be held in the frame of the project (multiplier events, seminars, transnational project meetings), should allow professionals from the health care and well fare systems to exchange views, experience and practices, while updating their knowledge through the participation of experts.3) reMind COMMUNITY: The core pillar of the project is the development of an innovative pilot and replicable community-based care ecosystem, to be powered by a network of public and private institutions and civil society representatives established much beyond project partnership. A Dementia Inclusive & Solidary Community, capable for embracing a social transformation proposal and qualified for comprehensively supporting caregivers and caretakers, thus involving the entire community in the inclusion and emancipation of all individuals.This result includes several integrated “services”, such as help-desk, helplines, information system, and a reMIND mobile App for caregivers and caretakers, among other. Further detail described under IO3;4) reMind SEEDS: Considering project’s pilot nature and being its rationale strongly representative of the needs of many other communities all over EU, a solid dissemination strategy is then fully justified, and should be considered as one of the main results of the project. Rights then enter into dramatized fairy tales and become an instrument of mediation of the value of rights through the direct experience of the elderly. Therefore, the way in which the elderly relate to society is renewed, giving them the role of promoters, narrators and witnesses in first person of the fundamental rights of society and their history penetrating into the world of children at school through fairy tales in their suitcases.The Rights project starts from a fundamental need: to make older people active again through doing, saying and imagining. The tool through which the project aims to achieve this are the suitcases of rights stories that will be built and brought to schools for children from 8 to 12 years. To do this, the elderly will have to know and analyze fairy tales, extrapolating and creating a discourse on human rights that will be reported as a narrative to the students. The elderly will benefit from specific training in the three areas: analysis of fairy tales and inviolable rights, the creation of suitcases for fairy tales of rights, class management methodologies in non-formal educational activities. This last part will see the direct collaboration of the teachers, who will also increase their skills by coming into contact with a new practical and concrete educational tool (as opposed to digital virtuality).The suitcases created will remain at the disposal of the local schools or cultural centres, which will act as a collection and exchange point for the suitcases of the fairy tales of rights and the training modules of the project.Through the commitment of the elderly, through the school and with activities aimed at the territory, Rights project also wants to involve society in a reflection on the inviolability and defence of human rights.The objectives of Rights are multiple:a) Creating a bridge between generations on the central theme of fundamental rightsb) To stimulate the elderly to resume an active role in society as promoters and defenders of fundamental rightsc) To train the elderly in the areas of defence of human rights, crafts and educationd) Raising awareness among the younger generations on the importance of defending their own and all society's rightse) To spread the importance of active citizenship through concrete actionsf) train the elderly to concretely realize and elaborate fairy tales in real suitcases to be exchanged and passed on to children in schools.The project will produce training modules and video lessons that will be available to all interested parties on the appropriate online platform. Furthermore, suitcases of fairy tales of rights will be made available to the local community through libraries and cultural centres and exchanged and distributed through the barter of suitcases. The impact of the project will therefore not be limited to the schools involved but will expand to the local and national community, acquire a transnational dimension thanks to the use of the internet.

The tools allow to understand different cultures present in EU countries. The project is implemented by a partnership of organizations and enterprises from Poland, Germany, Greece, Italy and Portugal.The game is created in the response to a reduced amount of educational materials enabling the development of intercultural competence of young people. There are practically no free tools to combine learning with other processes, i.e. communication, cooperation, leadership and conflict resolution. Based on the latest approaches to intercultural learning, the game will avoid stereotyping and the rote memorization of lists of do’s and don'ts, but in a contrary allows you to understand the intentions of the other person, not just their behavior.The game will be tested and evaluated by the Youth Testing Groups, which guarantee the production of an effective method that will be successfully used by youth workers in various EU countries, and in the same time young people will also be willing to use it.The educational tools will not only contribute to the development of intercultural competences of young people, but will also promote a new approach to the issue of intercultural communication, emphasizing the importance of dialogue and cultural exchange.Translation of the tools into the native languages of the countries participating in the project (Polish, German, Italian, Greek and Portuguese) will expand the group of users.All project activities drive to increase in the number and quality of workshops from the field of intercultural communication conducted in Poland, Germany, Portugal, Greece, Italy and other EU countries.

SparkSEEDs project aimed to build and develop a community based non-formal educational ecosystem with the aim to promote and support the regular offer of extra-school high quality and enriching activities, dedicated to and participated by youth, by capitalising important endogenous resources (human, intergenerational, cultural, natural, material, traditional, among other) in a crossed approach between youth main interests & needs and the most actual societal challenges. Powered by a network of public and private entities and civil society representatives, established beyond project partnership, a concrete “learning community” (SparkCOMMUNITY) was then created, competent for embracing a proposal for social transformation, and qualified for supporting the acquisition by youth of critical skills linked to the school, but going beyond its limits, thus involving the entire community in the formation of their individuals. Under the “umbrella” of the SparkCOMMUNITY a set of results, subsidiary & complementary, were achieved and contributed jointly, among other, for project global aims: > SparkYWorkers: A Capacity Building Programme (Trainings and Job Shadowing) for Youth Workers (facilitators, volunteers and NEETs) from the community & from its organisations, aiming to purposefully increase and adjust youth work skills and quality, thus giving capacity to assist regular & consistent non-formal learnings & activities for youth, and also to transfer societal fundamental values to young people, thus preventing the main phenomena actually affecting young people (school absence & unsuccess, bullying & any kind of violence, non-healthy behaviours, lack of participation & initiative, etc.); > SparkCLUBs: constituting the practical activities addressed to youth, the SparkCLUBs provided high-quality extra-school and free time activities for the integrated learnings of young people. The activities include as well the development of a SparkCAMP, a comprehensive Summer-Camp (programme of enriching activities for occupation of free time during school vacation times), facilitating an additional important result: safe & quality alternative/solution for families and educators for the ""positive"" occupation of the free time of their youngsters; > SparkSEEDing: A Dissemination Pack (as dissemination is one of the main pillars of the project), was developed in straight alignement with national, European and global trends on education. Besides, due to the pilot nature of the project and being project’s rationale strongly representative of the needs of many other educational ecosystems and communities all over Europe, a solid dissemination dimension was then fully justified by its usefulness and pertinence. SparkSEEDs partners enthusiastically believe that project proposal has the power to change positively the educational systems and issues addressed. This motivating idea was the compelling force behind all of SparkSEEDs products and results. With this in mind, and with the intention of supporting communities, its organisations and youth workers in their process of sparking the education of Youth, project should become a free and global interdisciplinary community based educational model, accessible for all. Along 24 months, a partnership composed by public and private institutions from Portugal, Italy and Poland, were responsible to address project objectives through the implementation of a sequent and interconnected plan of activities (SparkCOMMUNITY, SparkLABs, SparkCLUBs, SparkYWorkers Capacity Building Programme – 2 transnational trainings, 3 Job Shadowing actions, 5 Transnational Meetings, a set of Multiplier events, among other) could involve and benefit directly more than 400 youngsters (>180 with less opportunities) and more than 300 youth workers and educational staff. Indirect beneficiaries, namely through dissemination phase, communication measures and web based tools, a much larger number was achieved."

In straight connection, all partners agreed to build an European partnership (Portugal, Spain, Italy, Latvia and Scotland) that could address these common problems and gaps (that are generally not far from other national or European levels) seeking for so the following main objectives:> Supporting the production and adoption of Open Educational Resources, namely through the use of friendly, dynamic and widely accessible ICT, fostering the production of educational contents in different European languages, such as Latvian, Portuguese, Italian, Spanish and English;> Promoting the take-up of practical entrepreneurial experiences, initiatives, solutions and concepts in education and youth work, namely by supporting young people empowerment and concrete action in a crossed approach between educational needs, school curricular plans and respecting the full spectrum of youngsters’ ingenuity, creativity and particular interests;> Promoting the professional development of staff and youth workers, but as well the personal development of target groups, in ICT educational and pedagogic methodologies and tools, namely through the process of development and implementation of the CLICK model, but as well through the development and production of didactic contents to be spread under open educational resources;> Improving the attainment of young people, particularly those at risk of early school leaving and with low basic skills, by introducing innovative and more attractive educational resources in school and community context, which stimulates increasing levels of young people’s attention and learning motivation;> Promoting young people's social inclusion and well-being, besides knowledge and skills direct improvement, namely through the stimulation of empowerment, initiative, participation and leadership, but as well by contributing to solidify the school-community ecosystem (communities experiencing an active and open school, and school contributing to community enhancement).CLICK partners believe that simple practices can help changing attitudes and ultimately the regions in which youngsters are inserted. This underlying approach is the driving force behind all of CLICK’s products, including the CLICK web platform, the CLICK Teachings, the CLICK Clubs, and other CLICK initiatives focused in staff skills development. With this in mind, and with the intention of supporting teachers and learners around Europe, CLICK rely on a free global interdisciplinary educational model for teachers and learners. This model is also the home to CLICK Clubs, an exciting youth oriented program that aims to stimulate and praise the best ideas of students around Europe. Along 18 months period, the whole project activities (15 national and 4 transnational multiplier and training/learning events) benefited directly more than 500 youngsters, involving at least half with less opportunities, and a high number of youth workers, teachers and school staff (more than 300). Indirect beneficiaries, namely through communication measures and web based tools, a larger number of youngsters, youth workers and teachers was achieved.
